Four steps to care at home.
We walk beside you at every step, and we do the chasing so you do not have to.
1
Call us.
Tell us a little about the person who needs care. We listen first, in English or Somali.
- Who
- You and Mira

2
The assessment.
A certified MnCHOICES assessor from your county, tribe or health plan meets you to see what help is needed. CFSS also needs Medical Assistance or another Minnesota health care program; we help you check.
- Who
- Your county, tribe or health plan

3
Your service plan.
With a consultation services provider you write your CFSS plan: what help, how often, and who. You choose Mira as your agency and name your worker.
- Who
- You, with a consultation services provider

4
Care begins.
Your worker completes training and a background study, and we set the first visit with you.
- Who
- Mira and your worker

Questions families ask
Who pays for CFSS?
CFSS is paid through Medical Assistance or another Minnesota health care program. If you are not enrolled yet, we can tell you where to start.
How long does it take?
It depends mostly on when the assessment can be scheduled. We keep every step moving and tell you where things stand.
Can a family member be the worker?
Often, yes. Spouses and parents of minor children can be paid workers within limits set by DHS; a paid legal guardian cannot. We will look at your situation on the first call.
What if I have PCA today?
Minnesota has been moving people from PCA to CFSS at their assessments. If you still have PCA, call us and we will help you plan the switch.
Do you serve my area?
We serve Minneapolis, Hennepin County and the Twin Cities. If you live nearby, ask us anyway.
