These terms apply to your use of this website, run by Mira Homecare in Minneapolis, Minnesota. By using the website you agree to them. Services themselves are provided under a separate service agreement and your CFSS service delivery plan.
Information, not advice
The website explains our services and the CFSS program in general terms. It is not medical, legal or financial advice, and it does not decide whether you are eligible for any program. Eligibility for CFSS is decided by your county, tribe or health plan through an assessment, and by your Medical Assistance or other Minnesota health care program enrollment.
Not for emergencies
Do not use this website or its forms in an emergency. Call 911. Messages sent through the website are read during office hours.
What you send us
Please send only accurate information about yourself, and do not send health information, Social Security numbers or anyone else’s information without their permission. How we handle what you send is described in our privacy policy.
Acceptable use
Do not attempt to disrupt the website, to reach parts of it or of our systems you are not authorized to use, or to collect information from it by automated means. Staff and family sign-in areas are for authorized users only.
Our content
The text, design, logo and images on this website belong to Mira Homecare or are used with permission. You may share links to it and print pages for your own use.
